Improvement of the cooperation between cardiologists in resident physicians’ practices and stationary facilities - the patient in focus

• Implementation of communication guidelines between private practice and hospitals based on the “Chart of the collaboration between private practice and hospitals by the Swiss Society of Internal Medicine 02 www.sgim.ch
• Improvement of the implementation of guidelines in cooperation with hospitals, in part by optimising release strategies (Get with the guidelines programmes)
